Everyone else pays, and travel insurance is worth having for the repatriation cover rather than the treatment. Island clinics handle basics; anything serious means a transfer to Athens or Crete, which is quick but not instant.
Summer heat is the most common reason tourists need help — heatstroke and dehydration, not accidents.

Sunset drinking rather than clubbing, and priced accordingly. Fira has what late nights there are.

Neither Uber nor Bolt operates on the island and there are only about forty taxis for the whole of it. Book one through the hotel the day before, take the KTEL buses, or hire an ATV — which is what most people do.
